Project Request Forms

Long-term Projects

We require that faculty complete a Project Request Form for any long-term media development project (animation, video production, Web Site development, etc.). All sections of the form must be completed and the form returned to the DMC. The form will be presented to the Electronic Curriculum Faculty Advisory Committee for prioritization. Short-term Projects

All other projects such as slide scanning, document scanning or video digitizing, require the completion of the Digital Media Services Form . This form must be completed and signed when your materials are dropped off at the Digital Media Center. All DDS Students must have faculty sponsor sign the project form.
