The document is available in two formats.  The first format is a "pdf" where the Adobe Reader is needed to access the document.  If you do not have the reader already installed in your WEB browser you can click on the the Adobe Logo to download it now.
 
The document is also available in the MS WORD format and you will need to have WORD '95 or greater installed on your machine in order to access it.
Please note:  These are relatively large documents and may take substantial time to download over dial-up access.
