TASK FORCE: Statement of Rights and Responsibilities

University of Texas at San Antonio
* We are in the process of merging services and our informed consent mentions
that information may be released between services only according to treatment
needs.
Montclair State University
* we added a sentence or two in the rights and
responsibilities to let people know for informed consent purposes that we were
part of a merged unit and could at times share information between health and
counseling. On our end, we still feel the need to get a release, while the
health service is less inclined to do so unless we insist, which we do since we
know students don't always read the paperwork carefully, and we don't share
a physical location and are not necessarily viewed as the same unit by our
customers. Our legal counsel advised us that there was no legal reason we
couldn't share info without a release since we are part of the same unit, we
just feel it is good practice to do so unless there is an emergency. A front end
release is a good way of handling it and we do that at times, but not for every
client. Our records are totally separate and should be in my view although
collaboration is important.
University of Puget Sound
* We are a merged Counseling and Health Services office. We do include such a
statement in our Disclosure paperwork (required in this state) although we
maintain this particular notification is a courtesy as state law allows health
care providers to consult on a case without prior authorization to release
information.

* We have separate consent for treatment forms in health
& counseling, but each include a statement to the effect that members of the
staff will consult with each other about issues relevant to the students'
treatment. 2 years into this we haven't had any problems arise, no student
complaints, refusal to sign, etc.
