1.A recent, type written, complete Curriculum Vitae.
Your Curriculum Vitae must include the following information:
Full Name and degree
Home and office addresses
Phone and fax numbers
E-mail address, if available
Place and date of birth
Citizenship (visa status if in the US)
Education (degrees earned, year, name and location of schools)
Professional Licensure(s), year, state
Specialty Board Certifications (what field, year, country)
Employment history (institution, place, title for each position)
Present professional activities
List of Publications
Hobbies and special interest
2.The names of two members of HMAA as references.
